Flu vaccines are important to protect yourself, your family, and your community from flu. Each year on average about 8 percent of the U.S. Population get sick from the flu. The best way to prevent the flu is by getting the flu vaccine each year.

PLEASE NOTE: When you register online you will have a consent form created for you. If you would prefer to fill out a consent form manually please click here. If you manually fill out the form you will need to PRINT your consent form and bring it with you to your appointment. Please also bring your insurance card(s) with you to your appointment. Additionally, make sure to wear clothing that allows access to the upper arm/shoulder region.
