Please complete both forms below and bring to the Clinic

What: includes a blood pressure check and a blood draw for a cardiac risk profile which can assist you in identifying potential risk factors for cardiovascular disease. The profile includes readings for HDL, LDL, cholesterol/HDL ratio, glucose, triglycerides, and more. Waist measurements will also be taken. You are encouraged to share your results with your doctor.

Who: For state employees, spouses, retirees, and family members.

Instructions: This is a fasting blood test: no food for 12 hours prior to the test and no alcohol for 48 hours prior to the test. No doctor’s order is needed for this test.

Cost: Only $20; payment is due at the clinic.

Additional Tests: Big Savings!! Additional blood tests are available at a
savings with a Doctor’s Order. If you need a PSA or a thyroid screen, this could be an affordable option. Additional tests will be billed to your insurance provider and are eligible for insurance coverage. Review the list of additional blood tests available. Bring your Doctor’s Order with you to the clinic along with a completed Interpath Lab Form center section.

Results: Your results will be mailed to your home in a sealed envelope approximately one week after the Clinic. Use your results when you complete an online Personal Health Assessment.
